| BRIEF DESCRIPTION | A referral service for frail-aged, handicapped and younger disabled persons and their carers to assist such persons to remain in their own homes and avoid premature or inappropriate admission to hostels and/or nursing homes. Subject to funding limitations and appropriate assessment, subsidised referrals may be arranged to either of two visiting independent Podiatrists. |
